Website Terms of Use, Communications, Purchases, and Subscription Terms
Last Updated: August 24, 2026
These Terms govern access to and use of the websites, online forms, scheduling tools, patient-facing portals, applications, messaging programs, checkout pages, and other digital services that display or link to these Terms and are operated by or for Longevita Medical Institute, LLC (collectively, the “Digital Services”). In these Terms, “LMI,” “we,” “us,” and “our” refer to Longevita Medical Institute, LLC, except where a separate clinical or transaction document identifies another responsible entity.
1. Scope and Acceptance
The Digital Services are offered subject to these Terms. By accessing or using the Digital Services after these Terms are made reasonably available, you agree to the provisions that may lawfully be accepted through use. When LMI presents an unchecked box, “I Agree” button, or comparable affirmative-acceptance control, selecting that control constitutes your electronic signature and assent to the terms linked at that point.
A passive website visit, general inquiry, form submission, appointment request, or discovery call does not by itself authorize a charge, recurring subscription, marketing communication, prescription, procedure, or other clinical treatment. Those actions require the separate disclosures, consents, and approvals described in these Terms and in the applicable checkout, enrollment, or clinical documents.
If you do not agree to these Terms, do not use the Digital Services. You may contact LMI to request information or an alternative access method.
2. Eligibility and Minors
You must be at least 18 years old and legally capable of entering into a contract to purchase products, enroll in a subscription or membership, or provide consent through the Digital Services. A parent, legal guardian, or other legally authorized representative may act for a minor only when permitted by applicable law and LMI’s clinical and identity-verification procedures. The public-facing Digital Services are not directed to children under 13.
3. Separate Clinical and Transaction Documents
These Terms are not informed consent for medical care and do not replace any Notice of Privacy Practices, patient agreement, telehealth consent, procedure or medication consent, financial policy, cancellation or no-show policy, refund or return policy, membership agreement, direct health care agreement, order form, pharmacy or laboratory terms, or other service-specific document presented to you.
If documents conflict, the more specific document controls the subject it addresses. A Notice of Privacy Practices and any valid authorization control uses and disclosures of protected health information; a clinical consent or patient agreement controls clinical care; and an order form, checkout disclosure, or membership agreement controls the applicable purchase or recurring payment. Nothing in these Terms alters a licensed clinician’s professional standard of care or any nonwaivable patient right.
4. Medical Information and No Physician-Patient Relationship
Public website content is provided for general informational and educational purposes. It is not individualized medical advice and is not a substitute for professional evaluation, diagnosis, treatment, or emergency care. General content may not apply to your circumstances and may become outdated as evidence, products, and legal requirements change.
Browsing the Digital Services, submitting information, requesting an appointment, participating in a discovery call, receiving an automated output, sending a text or email, uploading a record, or requesting a product does not by itself create a physician-patient relationship. A physician-patient relationship is formed when a licensed clinician affirmatively accepts you for care and undertakes individualized professional services. Under LMI’s intended workflow, that ordinarily occurs at the first clinical appointment. LMI or its clinicians may review completed intake materials before an appointment for administrative or preparatory purposes; that preparatory review does not by itself signify acceptance for care. A relationship or professional duty may arise earlier, however, if a clinician undertakes individualized evaluation, advice, diagnosis, ordering, prescribing, treatment, or another professional service, or if applicable law otherwise provides.
Do not disregard professional medical advice or delay seeking treatment because of website content. Do not start, stop, or change any medication, supplement, treatment, or exercise program based only on public content or an unreviewed automated output. Individual results vary, and no outcome is guaranteed.
5. Medical Emergencies
The website, online forms, portal messages, email, and text messaging services are not monitored continuously and must not be used for an emergency or other time-sensitive medical need.
Call 911 or go to the nearest emergency department if you believe you may be experiencing a medical emergency. For an urgent but non-emergency concern, follow the contact instructions provided in your clinical documents and do not rely on a Digital Service for an immediate response.
6. Clinical Services, Telehealth, Portals, and Automated Tools
Clinical services are available only where LMI and the applicable licensed clinician are legally permitted to provide them and only after any required identity, location, eligibility, history, evaluation, consent, and payment steps are completed. LMI may determine that a requested service, medication, test, procedure, or telehealth encounter is unavailable or clinically inappropriate.
Telehealth has practical and technical limitations and may not be appropriate for every condition. You must provide your accurate physical location at the time of a telehealth encounter, use reasonably secure equipment and connectivity, and follow clinician instructions regarding in-person evaluation or emergency care. Portal and electronic messages may become part of the medical record where applicable.
Some Digital Services may use automated or artificial-intelligence-assisted tools to organize, transcribe, summarize, extract, calculate, or display information from questionnaires, records, laboratory reports, devices, wearables, or other sources. Automated outputs can be incomplete or incorrect. Unless a licensed clinician expressly reviews and confirms an output in the course of care, it is informational only and is not a diagnosis, prescription, treatment plan, or substitute for professional judgment.
Longevita Insights. Longevita Insights is a separately enrolled, self-directed, LMI-branded health-data and insights service delivered through a third-party technology platform. Enrollment in Longevita Insights alone does not establish a physician-patient relationship or include clinician monitoring, review, interpretation, diagnosis, medical advice, prescribing, treatment, emergency response, or care coordination. Data, scores, trends, alerts, summaries, reports, and artificial-intelligence-generated outputs available through Longevita Insights may be incomplete or incorrect and are provided for informational and educational purposes only. They are not reviewed or monitored by an LMI clinician as part of the Longevita Insights service. A clinician will review such information only if it is separately submitted and expressly accepted for review under a separate clinical relationship or service. LMI does not monitor Longevita Insights for urgent or abnormal findings. Do not use Longevita Insights for emergencies or as a substitute for professional medical care. Separate Longevita Insights enrollment terms, disclosures, privacy notices, acknowledgments, and consents also apply.
7. Accounts and Security
If an account is offered, you must provide accurate and current information, maintain one unique account and login for each authorized user, protect credentials and devices, and promptly notify LMI of suspected unauthorized access or a changed or reassigned telephone number. Do not share a patient-portal login or use another person’s account.
You are responsible for activity initiated through your authenticated account to the extent permitted by law. LMI may rely on instructions received through an authenticated account unless it has reason to believe the account has been compromised. LMI may require identity verification, reset credentials, or suspend access to protect users, LMI, or the Digital Services.
8. Communications
8.1 Transactional SMS and MMS Messages
By voluntarily providing your mobile telephone number through a designated opt-in process and requesting or agreeing to transactional communications, you consent to receive SMS or MMS messages from or on behalf of LMI at that number. Consent applies only to the categories disclosed at opt-in and does not authorize promotional or marketing messages unless you separately consent to them.
Transactional messages may include:
• Responses to inquiries
• Appointment scheduling
• Appointment confirmations and reminders
• Registration instructions
• Account or billing updates
• Care-coordination communications
• Customer-service communications
Message frequency varies. Message and data rates may apply.
You may revoke consent in any reasonable manner. You may reply STOP, QUIT, END, REVOKE, OPT OUT, CANCEL, or UNSUBSCRIBE to an incoming text. LMI will process a valid revocation within the time required by law. You may receive one nonpromotional message confirming the request or asking which category of messages you intended to stop.
Reply HELP for assistance, email info@LongevitaMI.com, or call 407-205-7877. If a messaging protocol does not permit replies, the message will identify a reasonable alternative opt-out method.
Consent to receive text messages is not a condition of purchasing or receiving medical services.
Mobile carriers are not responsible for delayed or undelivered messages.
LMI will not sell or rent text-messaging opt-in data or consent, or disclose them to unaffiliated third parties for those parties’ independent marketing. LMI may disclose such data to messaging platform providers, aggregators, wireless carriers, professional advisers, regulators, and other service providers as reasonably necessary to transmit, administer, secure, support, document, or comply with law concerning the messaging program.
8.2 Marketing Texts and Calls
Promotional or marketing texts and calls require a separate consent that identifies the sender, the telephone number to be contacted, the type of messages, and any automated or prerecorded technology used, as applicable. Marketing consent is voluntary, is not a condition of purchasing or receiving medical services, and may be revoked at any time. Consent to transactional messages is not consent to marketing.
An opt-out may stop all texts sent from the affected number or messaging channel, including operational texts, depending on the messaging system. If that occurs, LMI may communicate through another lawful channel for necessary service or care communications.
8.3 Text Message Privacy and Security
Standard SMS and MMS messages may not be encrypted. Text messaging may carry privacy risks, including the possibility that someone with access to your telephone or mobile account could view your messages.
Do not send highly sensitive medical, financial, credential, or identifying information by standard text message. Use an approved secure portal or another secure method when instructed. LMI may limit the content included in a text message and may require a secure channel before discussing sensitive information.
8.4 Email, Push Notifications, and Other Electronic Communications
LMI may send nonmarketing emails or notifications that are reasonably related to an inquiry, account, appointment, transaction, security event, or care relationship. Marketing emails will include an unsubscribe method where required. Opting out of marketing email does not prevent nonmarketing communications that are necessary to administer an account, transaction, or care relationship.
9. Purchases, Orders, Payments, and Prescription Requests
Where the Digital Services, including an electronic health record (“EHR”) or integrated payment function, permit a purchase, booking, deposit, or payment, LMI will present, before you submit payment information or authorize a charge, the amount to be charged, any recurring nature, taxes, shipping or other material fees, and the material cancellation, refund, return, rescheduling, and fulfillment terms applicable to the transaction. By completing the transaction, you authorize the disclosed charge through the selected payment method.
An order confirmation or payment receipt acknowledges receipt of a request; it does not guarantee acceptance, clinical eligibility, a prescription, product availability, a particular treatment, or a particular result. Orders and requests are subject to identity and payment verification, availability, applicable law, and any required clinician review. LMI may decline or cancel a request and will address any associated charge under the policy disclosed before purchase and applicable law.
Prices are stated in U.S. dollars unless otherwise shown. If a material pricing or description error is identified before fulfillment, LMI may correct the error and request your confirmation before proceeding or cancel the affected item and reverse the associated charge. LMI will not substitute a materially different product or materially increase a charge without disclosure and any consent required by law.
Prescription medications and prescription-only products require a valid prescription and professional review. A request for a medication is not a prescription. When a compounded drug is offered, it is not approved by the U.S. Food and Drug Administration, and FDA does not verify its safety, effectiveness, or quality before marketing. Any product-, medication-, procedure-, or service-specific disclosures and consents remain separately required.
Independent pharmacies, laboratories, payment processors, delivery services, technology providers, and other third parties may provide or fulfill parts of a transaction and may have separate terms and privacy practices. LMI is not responsible for an independent third party’s acts or omissions except to the extent responsibility cannot lawfully be limited.
THIS NOTICE MUST BE PROVIDED TO YOU UNDER FLORIDA LAW. This physician performs one or more stem cell therapies that have not yet been approved by the United States Food and Drug Administration. You are encouraged to consult with your primary care provider before undergoing any stem cell therapy.
10. Subscriptions, Memberships, and Automatic Renewal
Before enrolling a consumer in recurring billing, the enrollment page or signed agreement must clearly state the price, billing frequency, timing of the first charge, duration of any trial or introductory price, minimum commitment if any, material included and excluded services, automatic-renewal terms, cancellation method, and material refund terms. LMI will obtain a separate affirmative consent to the recurring charge and provide a record of the transaction.
Unless the applicable enrollment document states a fixed term or a different legally permitted rule, a month-to-month subscription renews each billing period until canceled. Cancellation stops future renewal and is effective as stated at enrollment; it does not retroactively reverse a properly disclosed charge or create a prorated refund unless the enrollment document or applicable law provides otherwise. A cancellation method may not be materially more burdensome than the method used to enroll, and an online enrollment must include an accessible online cancellation method where required by law.
LMI will provide any renewal reminder or notice of a material price or term change required by applicable law. Continued use will not be treated as consent to a new charge, arbitration provision, class-action waiver, or other material term when affirmative consent is legally required.
A membership or recurring fee does not guarantee acceptance as a patient, medical necessity, a prescription, a particular test or procedure, appointment availability, or a clinical result. Unless a separate agreement expressly states otherwise, a membership is not health insurance and does not replace health insurance.
If a membership constitutes a direct health care agreement under Florida law, LMI will use a separate written and signed agreement that states the covered services, fees, duration and renewal terms, termination rights, required refunds, and the legally required noninsurance disclosure. That agreement, not these general Terms, controls the direct health care arrangement.
11. User Submissions, Reviews, and Feedback
You represent that information you submit is accurate to the best of your knowledge and that you have authority to submit it. Do not place protected health information, financial credentials, or other sensitive information in a public review, testimonial, social-media field, or other publicly visible area.
If you voluntarily submit nonclinical content specifically for public posting, you grant LMI a nonexclusive, worldwide, royalty-free license to host, reproduce, display, and distribute that content for the requested purpose, subject to applicable law and any separate authorization requirements. LMI will not use identifiable patient information in advertising or a testimonial when a separate written authorization is legally required.
Suggestions and nonconfidential feedback may be used without compensation or attribution, but this clause does not transfer ownership of your medical records or authorize uses or disclosures of protected health information that are not otherwise permitted by law.
12. Acceptable Use
You agree not to:
• Use the website for unlawful purposes
• Attempt to gain unauthorized access to the website or related systems
• Introduce viruses, malicious software, or harmful code
• Interfere with website operation or security
• Misrepresent your identity
• Copy or misuse website content in violation of applicable law
• Use automated means to scrape, index, probe, test, overload, or extract data from the Digital Services except as expressly authorized
• Circumvent access controls, rate limits, security measures, or restrictions on a portal, account, subscription, or content
• Upload content that infringes another person’s rights, violates confidentiality, or contains unlawful, deceptive, or harmful material
• Use the Digital Services to obtain, divert, resell, or distribute prescription products or clinical services unlawfully
13. Intellectual Property
Website content, including text, logos, graphics, designs, photographs, and other materials, is owned by or licensed to Longevita Medical Institute and is protected by applicable intellectual-property laws.
Subject to these Terms, LMI grants you a limited, revocable, nonexclusive, nontransferable license to view and use the Digital Services for your personal, noncommercial use. You may not reproduce, distribute, modify, publicly display, reverse engineer, frame, mirror, create derivative works from, or commercially exploit protected content or software without prior written permission, except as applicable law expressly permits.
LMI names, logos, service names, and trade dress may be trademarks or service marks. No license to use any mark is granted by implication.
14. Third-Party Services and Links
The Digital Services may link to, embed, or interoperate with third-party websites, portals, applications, pharmacies, laboratories, payment processors, wearable-device platforms, scheduling services, or other vendors. LMI does not control their content, availability, security, terms, or privacy practices. A link or integration does not by itself constitute an endorsement. Your use of a third-party service is subject to that service’s terms and privacy notices, and LMI is not responsible for an independent third party’s conduct except to the extent applicable law provides otherwise.
15. Site Availability and Content Changes
LMI attempts to provide useful and current information but may correct, update, withdraw, or change public content, features, prices, and nonclinical offerings prospectively. Website content may contain typographical, technical, or other errors. A change to public content does not alter an already accepted paid transaction or clinical agreement except as that agreement and applicable law permit.
Digital Services may be unavailable because of maintenance, technical failure, cybersecurity events, third-party outages, force majeure, or other circumstances. LMI does not guarantee uninterrupted or error-free access and may modify or discontinue a nonclinical feature, subject to any contractual or legal obligations.
16. Disclaimer of Warranties
TO THE MAXIMUM EXTENT PERMITTED BY LAW, THE DIGITAL SERVICES AND PUBLIC, NONCLINICAL CONTENT ARE PROVIDED “AS IS” AND “AS AVAILABLE.” LMI DISCLAIMS IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, AND NON-INFRINGEMENT AS TO THE DIGITAL SERVICES. LMI DOES NOT WARRANT THAT PUBLIC CONTENT IS COMPLETE, CURRENT, ERROR-FREE, OR SUITABLE FOR YOUR PARTICULAR PURPOSE.
This Section applies to Digital Services and nonclinical content only. It does not disclaim or reduce professional duties, warranties, representations, or patient rights that cannot lawfully be disclaimed in connection with medical care, a paid transaction, or another applicable agreement.
17. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, FOR A CLAIM ARISING SOLELY FROM THE DIGITAL SERVICES OR PUBLIC, NONCLINICAL CONTENT, LMI AND ITS OFFICERS, MEMBERS, EMPLOYEES, CONTRACTORS, AND LICENSORS WILL NOT BE LIABLE FOR INDIRECT, INCIDENTAL, SPECIAL, EXEMPLARY, PUNITIVE, OR CONSEQUENTIAL DAMAGES, OR FOR LOST PROFITS, REVENUE, DATA, OR GOODWILL, EVEN IF ADVISED THAT SUCH DAMAGES WERE POSSIBLE. FOR SUCH A DIGITAL-SERVICES CLAIM, AGGREGATE DIRECT LIABILITY WILL NOT EXCEED THE GREATER OF $100 OR THE AMOUNT YOU PAID LMI FOR THE SPECIFIC DIGITAL SERVICE GIVING RISE TO THE CLAIM DURING THE 12 MONTHS BEFORE THE EVENT.
These limitations do not apply to professional negligence, gross negligence, willful misconduct, fraud, a violation of confidentiality or data-security duties that cannot be limited, infringement or misappropriation by LMI, personal injury or death to the extent caused by conduct for which liability cannot be waived, or any statutory or other right or remedy that applicable law does not permit the parties to limit. Some jurisdictions do not permit certain exclusions or limitations, so the provisions apply only to the maximum extent lawful.
18. Limited Indemnity
To the extent permitted by law, you will defend, indemnify, and hold harmless LMI and its officers, members, employees, contractors, and licensors from third-party claims, damages, judgments, and reasonable costs arising from your unlawful misuse of the Digital Services, material breach of these Terms, infringement of another person’s rights, or content you submit for public use. This obligation does not require you to indemnify any person for professional negligence, gross negligence, willful misconduct, or another liability that cannot lawfully be shifted to you.
19. Suspension and Termination of Digital Access
LMI may restrict or suspend Digital Service access when reasonably necessary to address suspected misuse, fraud, security risk, nonpayment, legal requirements, or material breach of these Terms. Where practicable, LMI will provide notice and an opportunity to cure. Terminating a website account or subscription does not by itself terminate an established physician-patient relationship, and termination of clinical care must follow the applicable clinical documents, professional duties, and law.
20. Privacy, HIPAA, and Data Use
LMI’s website Privacy Policy describes collection, use, disclosure, retention, and choices for personal information handled through the public Digital Services. LMI’s separate Notice of Privacy Practices describes permitted uses and disclosures of protected health information by a HIPAA-covered component, where applicable. These Terms are not a HIPAA authorization and do not authorize any use or disclosure for marketing, sale, research, or another purpose when a separate written authorization or other legal basis is required.
https://www.longevitamedicalinstitute.com/privacy-policy
The Notice of Privacy Practices is available through the Digital Services or upon request.
LMI may use information in de-identified or aggregated form for lawful operations, analytics, quality improvement, and statistical purposes. LMI will not use identifiable patient information to market goods or services, sell such information, or disclose it for another party’s independent marketing unless applicable law permits the use without authorization or LMI first obtains any required specific written authorization.
Cookies, pixels, analytics, session-replay tools, and other tracking technologies may collect information about use of the Digital Services. Their use and available choices must be accurately described in the Privacy Policy and implemented consistently with applicable consent, HIPAA, contracting, and security requirements. A privacy notice, cookie banner, or acceptance of these Terms is not by itself authorization to disclose protected health information to an analytics or advertising vendor.
No electronic transmission or storage method is completely secure. Use the approved secure portal for sensitive information when available, protect your credentials and devices, and promptly report suspected unauthorized access. LMI’s security and incident-response obligations are governed by applicable law and are not reduced by this statement.
21. Changes to These Terms
LMI may revise these Terms prospectively. The revised version will identify its “Last Updated” date. LMI will provide additional notice of a material change when required by law or when reasonably practicable, and will obtain renewed affirmative assent when a change creates a new recurring charge, materially changes a paid service, adds a dispute-resolution restriction, or otherwise requires new consent. Changes do not retroactively alter accrued rights, completed transactions, or clinical obligations.
22. Governing Law and Venue
These Terms are governed by Florida law, without regard to conflict-of-law principles. Except for a clinical claim, a claim subject to a mandatory statutory venue, or a matter that cannot lawfully be restricted, an action arising solely from the Digital Services or these Terms must be brought in a state court located in Pasco County, Florida, or in the United States District Court for the Middle District of Florida, and each party consents to personal jurisdiction there.
23. Miscellaneous
If a provision is held unenforceable, it will be enforced to the maximum lawful extent and the remaining provisions will remain in effect. A waiver must be explicit and does not waive a later breach. LMI may assign these Terms in connection with a merger, reorganization, financing, sale of assets, or transfer of the Digital Services, subject to applicable privacy and clinical obligations; you may not assign them without written consent. Neither party is liable for delay caused by events beyond reasonable control, except for payment obligations and duties that applicable law does not excuse.
These Terms, together with the documents expressly incorporated for a particular Digital Service or transaction, are the entire agreement concerning general use of that Digital Service. They are not the entire agreement concerning medical care. Headings are for convenience. Electronic records and signatures may be used to the extent permitted by law. Provisions that by their nature should survive termination, including intellectual property, disclaimers, limitations, indemnity, governing law, and accrued payment obligations, will survive.
24. Accessibility
LMI seeks to provide effective access to its Digital Services for people with disabilities. To report an accessibility barrier or request a reasonable auxiliary aid, service, or alternative method of access, contact info@LongevitaMI.com or 407-205-7877. Include the page or feature involved and the preferred method of response. Accessibility requests will be handled separately from emergency or time-sensitive medical communications.
